Job summary

One Community Health in Sacramento, Midtown Clinic, is seeking a Behavioral Health Clinician I to provide individual, family, and group counseling services to patients and their families.

The ideal candidate holds a Master’s degree in Social Work, state ASW certification, CPR/BLS, and is bilingual (English plus Spanish, Dari, or Pashto). You will work in a multidisciplinary team, develop treatment plans, and coordinate care within integrated behavioral health services.

Qualifications

• Master's degree in Social Work.
• State Certification as Associate Clinical Social Worker (ASW).
• Bilingual, English plus Spanish or Dari or Pashto.

Responsibilities

• Conduct individual and group therapy sessions.
• Develop personalized treatment plans with patients and discharge goals.
• Collaborate with physicians, nurses, case managers, and other therapists to coordinate care.
• Refer patients for additional psychological or psychiatric services as needed.
• Participate in integrated systems of care and multidisciplinary team meetings.
