Position Title : Behavioral Health Case Manager III
Location: Bridgeville, Delaware
Schedule: Monday through Friday 8AM-4:30PM
Pay Rate: $32-$33
Supervision Received:
- Reports to the Clinical Supervisor. Individual supervision is provided at the Clinical Supervisor’s discretion.
Duties:
- Interviews clients, family members, and/or persons deemed necessary to obtain the most complete case history.
- Oversees the operation of PDP staff at the assigned State Police Location.
- Provides technical guidance to professional and para-professional staff, and monitors co-workers to assure a quality level of services. Trains and/or orients new staff.
- Maintains a caseload, providing therapeutic treatment to clients and families.
- Provides consultation to police to enable them to intervene in crises or directly handle explosive situations.
- Participates in inter-disciplinary team conferences to help the program grow.
- Prepares narrative and statistical reports on a weekly and monthly basis.
- Provides consultative services to State and private agencies and may participate in community events led by the State Police.
- Makes home visits with a peer to assess a client’s needs.
- Coordinates treatment and other services within 24 hours of the client asking for help, which is essential.
Requirements:
- Bachelor’s Degree in a related field, Master’s Degree preferred.
- Three years’ experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs using effective counseling or interviewing techniques, as well as other available resources such as service plans or case management systems.
- Three years’ experience in providing clinical treatment, counseling, or determining eligibility for services or benefits as part of a treatment team.
- Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
- Six months experience in narrative report writing.
- Must be able to communicate clearly with clients, providers, AND POLICE.
- Must be able to keep appropriate documentation.
- Be a self-starter and willing to learn.
- Valid Driver's license.
