Responsibilities

• Screen new service requests and ensure appropriate service according to policies.
• Attend interdisciplinary staff meetings and manage caseload assignments.
• Complete behavioral health assessments with clinical oversight.
• Develop treatment/service plans and provide direct client services (case management, counseling, transportation, etc.).
• Provide ongoing support to achieve service plan goals; conduct regular face-to-face reviews.
• Assist with tracking clients and generating reports for quality of care.
• Document all service contacts timely and maintain case records per standards.
• Maintain communication with consulting psychiatrist regarding medication changes.

Completes case management assessment on all clients entering caseload* Develop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clients* Provide direct client services to individuals and/or family members. Services provided may include case management, skills training, group skills training, health promotion, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need. Services may be provided in office, home, school or community settings* Provide on-going supportive and/or case management functions in accordance with the problems, needs and the strategies identified within the service plan in order to assist the client to achieve goals and objectives* Provide face to face reviews with the client and/or relevant behavioral health staff on a regular basis. Ensures progress made is reported so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved.
