Job summary

As a Behavioral Health Care Coordinator at Talkiatry, you will support patients through enrollment, ongoing engagement, and discharge while collaborating with a multidisciplinary

Qualifications

• Bachelor's degree in social work, psychology, counseling, public health, or a related field.
• At least three years of behavioral health case management, care coordination, or discharge planning experience.
• Experience with higher-acuity populations and multidisciplinary clinical teams.

Responsibilities

• Support patients through enrollment, ongoing engagement, discharge, and step-down care.
• Conduct outreach to identify barriers and help navigate benefits, transportation, and community resources.
• Coordinate handoffs with psychiatrists, therapists, and external partners.
