Job summary

ViaQuest, Inc. seeks a Behavioral Health Case Manager to support children, adolescents and adults in our behavioral solutions teams in the Toledo area.

You will collaborate with licensed clinicians to deliver Therapeutic Behavioral Services and help develop individualized treatment plans. Responsibilities include coordinating care, linking clients to services, crisis prevention and promoting positive relationships with external contacts.

Qualifications

• Must be 18 years or older.
• Bachelor's or Master's degree in a related field or a high school diploma with 3+ years of relevant field experience.
• Experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel throughout assigned service area.

• Develop service planning for individualized supports or care coordination of healthcare and behavioral health services.

• Promote a positive and cooperative relationship with all outside contacts.
