Job summary

Pathways Inc. in Kentucky is seeking a full-time Case Manager to provide case management services to assigned consumers in alignment with individualized treatment plans.

You will coordinate services across behavioral health, medical, social, and community resources, while maintaining thorough, compliant documentation and collaborating with families, providers, and community partners to ensure continuity of care.

Qualifications

• Bachelor's Degree in Human Services or a related field is required.
• Minimum of one year of post-graduate experience required.
• Experience in behavioral health preferred.
• Proficient in maintaining accurate documentation and using computers/EHR.

Responsibilities

• Provide case management services to assigned consumers in accordance with individualized treatment and service plans.
• Coordinate and monitor services to support consumers' behavioral health, medical, social, and community resource needs.
• Maintain accurate, timely, and current documentation in compliance with agency, state, and federal requirements.
• Collaborate with consumers, families, providers, and community partners to ensure continuity of care.
• Assist consumers with accessing community resources, benefits, and supportive services.
• Participate in treatment team meetings and contribute to care planning efforts.
• Adhere to all Pathways policies, procedures, and confidentiality standards.
• Perform additional duties as assigned within the scope of the position.
