Position Summary:

The Behavioral Health Case Manager, as part of the Philadelphia Behavioral Health System, works as a member of a multi-disciplinary team that provides short-term outreach services to consumers (adults and children) and families with high acuity behavioral health needs. The Case Manager collaborates with other service providers to ensure coordination among physical health, behavioral health, and social services. The primary role involves case coordination and care planning by assisting consumers and families in navigating various systems.

Duties and Responsibilities:
  1. Assess strengths and functioning levels to develop client-centered and family-focused plans, consulting with social services and hospitals to ensure comprehensive care.
  2. Participate in inter-agency planning meetings.
  3. Provide outreach and engagement to high-need consumers and families.
  4. Coordinate behavioral health care services from assessment through treatment, ensuring seamless service delivery.
  5. Assist consumers and families in navigating multiple service systems for stabilization and community reintegration.
  6. Link and advocate for consumers and families with social, educational, vocational, and health services, ensuring culturally appropriate support.
  7. Support community living by establishing and maintaining community connections.
  8. Assess needs, develop skills, resolve problems, and facilitate access to long-term services and entitlements.
  9. Keep updated on available resources and access procedures.
  10. Advocate for consumers to access necessary services and supports.
  11. Perform additional tasks as assigned and maintain flexibility in work schedule.
  12. Bilingual (English/Spanish) skills are strongly recommended but not mandatory.
Education and Experience:
  • Bachelor’s Degree in a recognized Behavioral Health field and at least two (2) years of experience in mental health or substance abuse services, preferably in community-based treatment.
  • Experience as a treatment team member, with preference for those familiar with children’s social services in Philadelphia.
  • Valid driver’s license and security clearance required.
