Job summary

Centerstone is seeking a dedicated Case Manager to coordinate care for patients across clinic, home, and community settings. The role emphasizes teaching the connection between physical and mental health, with a focus on whole-person recovery.

Ideal candidates hold a bachelor’s degree in counseling, psychology, or social work and are passionate about helping people cope with mental health challenges. A valid driver’s license and EHR experience are required.

Qualifications

• Bachelor’s degree in psychology, social work or related field is required.
• Experience with Electronic Health Record is preferred.
• Valid driver’s license in good standing is required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate behavioral and primary care needs and develop wellness strategies.
• Refer and link clients to additional services as identified.
• Develop and update treatment plans with client and family input.
• Enter documentation into the electronic medical record in a timely manner.
