Job summary

Advocate Health's Behavioral Health Services seeks a qualified discharge planner to coordinate comprehensive discharge plans for child/adolescent units. You will work with families, physicians, and care teams to ensure timely, appropriate care and education about benefits and options.

Must have a Master’s in Social Work or Psychology Counseling, licensure (LCSW/LCPC IL), and at least 3 years in behavioral health, with strong assessment, utilization review, and reimbursement knowledge.

Qualifications

• Demonstrated leadership in behavioral health settings.
• Strong written and verbal communication skills.
• Knowledge of reimbursement methods and referral sources.
• Understanding of State and federal mental health/chemical dependency regulations.
• Experience with discharge planning and utilization review in managed care.

Responsibilities

• Coordinate discharge plans and educate families, ensuring timely completion.
• Explain benefits and assist patients in exploring financial options.
• Obtain pre-authorization and monitor ongoing reviews with payors.
• Initiate denial/appeal processes for non-certified care and complete required paperwork.
• Support development and maintenance of case management standards across Behavioral Health Services.
