SUMMARY
The primary responsibility of the Certified Behavioral Health Care Manager II is to ensure the implementation of the comprehensive care plan, which includes mental health goals, physical health goals, and other life domain goals to achieve clinical outcomes aligned with the member's needs and preferences. Care Managers regularly interact with staff, the Health Team, and treatment providers to assess and treat individuals with mental, emotional, or substance abuse problems, including alcohol, tobacco, and other drugs. Activities may include crisis intervention, case management, treatment planning, client advocacy, prevention, and education. As part of the Health Team, the Behavioral Health Care Manager may provide ongoing service coordination and link members to resources following appropriate training.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Provides practical help, support, advocacy, coordination, individualized support, problem solving, direct assistance, training, and supervision to help consumers obtain necessities of daily living, including medical and dental care, legal and advocacy services, financial support such as entitlements (e.g., adult foster care, paid roommates, meals), money-management services (e.g., payeeships), and transportation.
- Participates in disposition/treatment team meetings, providing pertinent information related to the consumer's treatment.
- Assists in developing, writing, implementing, evaluating, and revising Service Plan goals and objectives.
- Coordinates approved services, facilitates communication between service providers and DHS or other referral sources, advocates for consumers, and assists in arranging follow-up support services.
- Educates and supports consumers’ families.
- Assists consumers in accessing Primary Care services and supports learning self-management skills for physical health concerns, including preventive care.
- Maintains core competencies related to working with co-occurring disorders through continuing education and applying skills in treatment.
- Completes all paperwork and prior authorizations, including Service plans, Consumer Assessment Records, and progress notes within designated timeframes.
- Documents consumer progress to maintain records according to established methods.
- Provides ongoing assessment of mental health symptoms, response to treatment, and adjusts treatment plans as needed to ensure safety and effectiveness.
- Ensures compliance with Medicaid, ODMHSAS, CARF, and other funding or contractual requirements.
- Maintains confidentiality, informs consumers of rights, and obtains necessary authorizations for information sharing.
- Provides coverage during co-worker absences as needed.
- Delivers direct clinical services to consumers individually, in groups, and families, in office and community settings, teaching behavioral management techniques and promoting growth.
- Coordinates with inpatient services for detoxification and linkage to self-help programs and residential facilities.
- Builds partnerships with government agencies and promotes community education on PTSD and mental health crises.
- Supports and facilitates Care Plans within the Red Rock Health Team.
- Performs other duties as required.
QUALIFICATIONS
- Certified as a Behavioral Health Case Manager II (BHCM II) or meet alternative parameters such as a relevant degree and experience as specified.
- Bilingual skills preferred.
- Demonstrates independent judgment, tact, patience, and effective relationship-building with consumers, staff, and community organizations.
- Completes required training and maintains basic computer skills.
- At least two years of experience working with community services, health agencies, and low-income or minority consumers, with an understanding of cultural factors.
