Spurwink is seeking a collaborative care coordinator (case manager) in the greater Portland area to support adults with behavioral health challenges. The role requires MHRT/C or eligibility and offers an interdisciplinary team approach to planning and care delivery.

The position emphasizes intake and ongoing assessments, development of integrated care plans, and close collaboration with clinical leaders, nursing staff, and medical consultants.

Qualifications

• Bachelor's degree preferred and relevant social services background.
• MHRT/C certification or eligibility is required or anticipated.
• Valid driver's license with clean driving record.

Responsibilities

• Provide intake/assessment when referred and ongoing assessment as required.
• Facilitate team meetings and develop a care plan with input from the team.
• Oversee implementation of care planning and monitor progress.
• Collaborate with Clinical Team Leader, Peer Partner, Nurse Care Manager, and consultants.
