This full-time position requires attendance during evening shifts. The Care Coordinator will work to enhance client experiences by promoting access to supportive services.

Formación

• Ability to establish supportive client relationships.
• Lived experience with the Behavioral Health system is preferred.
• Ability to perform duties in a stable and predictable manner.

Responsabilidades

• Coordination of outreach and engagement efforts.
• Develop strategies to improve client attendance to appointments.
• Provide phone and face-to-face follow-up to clients.
