Elevate Mental Wellness is a growing outpatient behavioral health practice seeking an experienced, dependable, and highly organized Care Coordinator for our Englewood location.
The Care Coordinator serves as one of the primary points of contact for patients, providers, referral sources, pharmacies, insurance companies, and other members of the care team. This position plays an important role in ensuring that patients receive timely, coordinated, and compassionate support throughout their care.
This is primarily an administrative and patient-coordination position, but it also includes limited clinical support responsibilities such as obtaining vital signs and assisting with drug testing when needed.
The Care Coordinator works primarily onsite at our Englewood office while also helping provide administrative support to our other clinic locations. Because we serve individuals with a variety of behavioral health needs, the work environment can be fast-paced and occasionally unpredictable. The ideal candidate is comfortable managing competing priorities while maintaining professionalism, accuracy, confidentiality, and compassion.
Key Responsibilities
Responsibilities include, but are not limited to:
- Answering incoming phone calls and assisting patients with scheduling, general questions, and requests
- Routing calls, messages, and clinical concerns appropriately
- Monitoring and responding to patient portal messages, voicemails, faxes, and assigned tasks
- Scheduling, rescheduling, and confirming appointments
- Managing provider calendars and helping maintain accurate schedules
- Greeting patients and supporting day-to-day front desk operations
- Registering new patients and verifying that required intake information and documentation are complete
- Reviewing patient demographic and insurance information for accuracy
- Supporting new-patient intake and referral workflows
- Assisting with insurance eligibility verification and identifying coverage concerns
- Working with Medicaid and other insurance plans and understanding basic insurance terminology, including copays, deductibles, prior authorizations, eligibility, and covered services
- Assisting providers with prior authorization requests for medications and services
- Coordinating referrals to outside providers and community resources
- Requesting and tracking medical records
- Obtaining releases of information and understanding the importance of appropriate authorization before releasing protected information
- Communicating with pharmacies, medical offices, hospitals, therapists, case managers, probation/parole officers, county agencies, and other members of a patient's care team when appropriate
- Documenting patient communications, outreach attempts, care coordination, and follow-up activities accurately in the electronic health record
- Reviewing assigned tasks and ensuring that follow-up is completed and documented
- Tracking outstanding referrals, records requests, prior authorizations, and patient needs through completion
- Assisting with hospital discharge and continuity-of-care coordination
- Supporting providers with routine administrative and clinical follow-up
- Obtaining and documenting vital signs, including blood pressure, pulse, weight, height, and oxygen saturation, as applicable
- Assisting with urine or oral-fluid drug testing and related documentation when needed
- Collecting patient balances and communicating professionally regarding copays, deductibles, and payment responsibilities
- Maintaining confidentiality and complying with HIPAA and applicable behavioral health privacy requirements
- Working collaboratively with providers, therapists, case managers, administrative staff, and leadership
Documentation Responsibilities
Accurate documentation is an important part of this position.
The Care Coordinator must be comfortable documenting patient-related activities in an electronic health record. Documentation may include:
- Patient outreach attempts
- Appointment and scheduling communications
- Referral follow-up
- Insurance and prior authorization communications
- Records requests
- Communication with outside providers or agencies
- Transportation or resource coordination
- Patient concerns and escalation to clinical staff
- Follow-up tasks and outcomes
Documentation must be clear, objective, timely, and accurately reflect what occurred. Because documentation becomes part of the patient's medical record, attention to detail and understanding the difference between administrative communication and clinical information are essential.
Preferred Experience
We strongly prefer candidates with previous experience in behavioral health, healthcare, medical administration, case management, care coordination, or a related setting.
Experience with any of the following is especially helpful:
- Behavioral health or mental health services
- Medicaid
- Medicare and/or commercial insurance
- Electronic health records
- Medical or behavioral health documentation
- Writing patient communication or care coordination notes
- Insurance eligibility verification
- Referrals and records requests
- Scheduling in a medical or behavioral health practice
- Working with pharmacies and prescribes
- Working with hospitals, county agencies, probation/parole, social services, or community organizations
- Substance use treatment services
- HIPAA and confidentiality requirements
- Patient collections, copays, and deductibles
Qualifications
The ideal candidate:
- Has previous healthcare, behavioral health, medical office, case management, or care coordination experience
- Understands the importance of accurate medical-record documentation
- Has experience working with Medicaid or is familiar with Medicaid processes
- Is comfortable learning insurance requirements and healthcare terminology
- Has strong written communication skills and can write clear, professional documentation
- Is comfortable using electronic health records and learning new software systems
- Can manage multiple tasks, messages, calls, and deadlines without losing track of follow-up
- Pays close attention to details and catches inconsistencies or missing information
- Understands when a concern should be escalated rather than attempting to resolve something outside their role
- Demonstrates good judgment and respects professional boundaries
- Is dependable, punctual, and consistent
- Takes ownership of assigned responsibilities
- Follows established procedures rather than creating their own process without approval
- Accepts feedback and applies coaching to future work
- Can prioritize effectively when several things require attention at the same time
- Remains professional and composed when patients are frustrated, distressed, or emotionally escalated
- Communicates respectfully with patients, coworkers, providers, and outside agencies
- Is comfortable working independently while also functioning as part of a team
- Understands that behavioral healthcare requires compassion while also maintaining appropriate boundaries and accountability
This is an onsite position at our Englewood clinic.
The environment is collaborative, supportive, and team-oriented, but behavioral healthcare can be busy and unpredictable. Priorities may change throughout the day based on patient needs, provider requests, urgent concerns, hospital discharges, insurance issues, or other operational needs.
Candidates should be comfortable working in an environment where flexibility, communication, and the ability to prioritize are important.
