Job summary

Prisma Health is seeking a licensed behavioral health clinician to join the Behavioral Care Day Treatment team. The role involves performing biopsychosocial assessments, creating individualized treatment plans, and delivering individual, family, and group therapy for patients with mental health and substance use concerns.

Responsibilities include case management, ensuring appropriate level of care, documentation, and collaboration with a multidisciplinary team.

Qualifications

• Master's degree in Social Work, Counseling, Psychology or related field.
• Currently licensed LMSW or LPC-A or LMFT-A in the state.
• Experience with group, individual, and family therapy; able to assess and plan effectively.
• Proficient with electronic medical records and documentation.

Responsibilities

• Perform biopsychosocial assessments and develop individualized treatment plans.
• Provide individual, family, and group therapy to adults, adolescents, and families.
• Coordinate case management activities and communicate with referral sources.
• Monitor outcomes and participate in multidisciplinary treatment planning.
• Perform additional duties as assigned.

Job Summary

Performs biopsycho-social assessments, creates and updates individualized treatment plans, provides individual, family and group therapy for patients to Outpatient Behavioral Care Services. Specializes in work with adults, and/or adolescents, and families experiencing mental health/substance abuse problems.

Essential Functions

• Completes an accurate biopsychosocial assessment, including competency in diagnostic determinations and determination of appropriate level of care for patient.
• Provides direct and indirect clinical/therapeutic services to patients and their families with clinical supervision as needed.
• Develops and individualized treatment plan including measurable treatment goals related to patient's presenting problems. Participates as an active member of the multi-disciplinary treatment plan. -
• Provides the most appropriate therapy, specific to patient's symptoms.
• Performs any case management responsibilities involved with direct patient care. This includes, but is not limited to, Utilization Review, Discharge Planning, Family Involvement/Education, timely and appropriate documentation of care provided, communication with referral sources, completion of short-term disability paperwork and communication with patient's employers as well as coordination of care with multidisciplinary treatment team and outside community providers.
• Participates in outcome monitoring activities.
• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - Master's degree in Social Work, Counseling, Psychology or related human services field of study.

• Currently licensed as a Licensed Master Social Worker (LMSW) in the state the team member is working, or currently licensed as a Licensed Professional Counselor-Associate (LPC-A) in the state the team member is working, Marriage and Family Therapists and Psycho-Educational Specialists, or currently licensed as a Marriage and Family Therapist-Associate (LMFT-A) in the state the team member is working.
