AdventHealth in Lake Mary, FL is seeking a Clinical Social Worker (LCSW/LMHC) to coordinate care and advance whole-person health. You will develop therapeutic plans, intervene in crises, and collaborate with physicians to ensure integrated care and resources for patients.

The role requires a Master's in a related field and 2+ years of clinical experience in behavioral health, outpatient or home health settings. Florida licensure is required or eligible.

Job Description:

• Prioritizes whole person health problems, formulates goals, and constructs therapeutic plans, revising as needed based on continuous evaluation and assessment of progress.
• Quickly appraises crisis situations and selects appropriate interventions.
• Mediates highly complex situations and develops treatment plans with minimal supervision.
• Acquires working knowledge of motivational interviewing and works with complex clients.
• Seeks appropriate consultation when necessary.
• Addresses participants' behavioral health as it contributes to chronic disease.
• Identifies modifiable social determinants and works with participants to modify them.
• Collaborates with referring physicians to navigate participants through an integrated referral network to identify necessary resources for physical and behavioral health diagnoses.
• Communicates progress of participants to referring physicians.
• Evaluates practice upon completion of case intervention to determine success and achievement of expected outcomes.
• Prepares and teaches program classes.
• Identifies necessary resources for patient referral to achieve whole-person health and increased stability in daily living.
• Performs other duties as assigned.
