Job summary

AdventHealth in Lake Mary, FL is seeking a Behavioral Health professional to develop and implement holistic care plans for participants. You will assess crises, coordinate with physicians, and guide patients toward resources that support whole-person health.

The role requires a Master’s degree and LCSW/LMHC licensure, with 2+ years of clinical experience in outpatient or home health settings. You will work in a team, manage a busy caseload, and mentor patients through recovery and stability.

Qualifications

Master's degree required in Behavioral Health, Psychology, Social Work or related field.
2+ years nursing, social work or related clinical experience in a healthcare setting.
Licensure as LCSW or LMHC is required.

Responsibilities

Prioritizes whole person health problems, formulates goals, and constructs therapeutic plans, revising as needed based on progress.
Quickly appraises crisis situations and selects appropriate interventions.
Mediates highly complex situations and develops treatment plans with minimal supervision.
Acquires working knowledge of motivational interviewing and works with complex clients.
Collaborates with referring physicians to navigate participants through an integrated referral network to identify resources.
Communicates progress of participants to referring physicians.
Evaluates practice upon completion of case intervention to determine success and outcomes.
Prepares and teaches program classes.
Identifies resources for patient referral to achieve whole-person health.

Addresses participants' behavioral health as it contributes to chronic disease.
Identifies modifiable social determinants and works with participants to modify them.
Collaborates with referring physicians to navigate participants through an integrated referral network to identify necessary resources for physical and behavioral health diagnoses.
Communicates progress of participants to referring physicians.
Evaluates practice upon completion of case intervention to determine success and achievement of expected outcomes.
Prepares and teaches program classes.
Identifies necessary resources for patient referral to achieve whole-person health and increased stability in daily living.
