Job summary

Frederick Health seeks a Full-time Clinical Pharmacy Specialist to work Monday - Friday, 8:00a - 4:30p, every 4th weekend. This role delivers real-time clinical pharmacy services to optimize medication therapy and patient safety in a high‑acuity environment.

The position emphasizes collaboration with physicians, nurses and the care team, participation in rounds and emergency responses, and precepting pharmacy students and residents. Eligibility includes PharmD, PGY1, and state licensure.

Qualifications

• PharmD degree or equivalent.
• Completion of PGY1 Pharmacy Residency or equivalent clinical experience.
• Maryland Pharmacist license or eligible to reciprocate.
• ACLS, PALS and/or BLS certification within one year required.

Responsibilities

• Provide bedside pharmacotherapy expertise for patients across all acuity levels.
• Perform prospective medication order review and intervene to prevent medication errors, adverse drug events, and therapeutic delays.
• Conduct medication reconciliation for high‑risk or complex patients on arrival.
• Oversee medication distribution, inventory management and controlled substances oversight.
• Participate in quality improvement initiatives and outcome metrics reporting.
