Frederick Health is seeking a Full-time Clinical Pharmacy Specialist to provide real-time clinical pharmacy services in a high acuity setting. Qualifications

• Doctor of Pharmacy (PharmD) or equivalent
• Completion of PGY1 Pharmacy Residency or equivalent clinical experience
• Maryland Pharmacist license or eligible to recipro­cate
• ACLS, PALS and/or BLS certification within one year (required)

Responsibilities

• Provide bedside pharmacotherapy expertise across all acuity patients.
• Perform prospective medication order review to prevent errors.
• Conduct medication reconciliation for high-risk patients on arrival.
• Optimize therapy with dose adjustments and antimicrobial stewardship.
• Participate in rounds, huddles, and interdisciplinary discussions.
• Assist with precepting students and residents.
• Respond to code blues and rapid response events.
• Oversee medication distribution, inventory, and CSA oversight.
• Support quality improvement initiatives and outcome reporting.
