Job summary

Bluegrass Care Navigators seeks a PACE Transportation Driver to provide direct transport for participants between home, the PACE Center, and medical providers. You will interact with participants, caregivers, and the PACE team daily.

This PRN role serves Lexington and Frankfort regions, requiring a friendly, organized driver with a current Kentucky license and CPR certification.

Qualifications

• High school diploma or GED required.
• Must be at least 18 with a Kentucky license and 2 years of driving experience.
• Minimum 1 year of transporting passengers.
• Experience with mobility-impaired passengers or willingness to train.
• Experience with frail or elderly populations or willingness to train.
• Current CPR certification.
• Proficient reading and writing.
• No more than 2 moving violations in 36 months; no DUI in 5 years.
• Able to interact effectively with participants, caregivers and staff.
• Able to keep accurate records; well organized.
• Entry-level computer skills required.
• Must be a licensed driver.

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Service Coverage:

This role is responsible for providing services in both the Lexington (LEX) and Frankfort (FFT) regions

Position Summary:

The PACETransportationDriver is responsible for the direct transportation of PACE participants to and from their destinations, including home, the PACE Center, medical providers, etc. Position requires constant interaction with PACE participants, caregivers, community agencies, medical providers and PACE program staff and leadership.

Position Qualifications:

• High school diploma or general education degree (GED) required .
• Must be at least 18 years of age and have a current Kentucky driver’s license, witha minimum of twoyears’driving experience.
• At least one yearofworkexperience in transportingpassengers.
• Minimum of oneyear’sexperience transporting individuals with special mobility needs or must receive appropriate training upon hire.
• Minimum of oneyear’sexperience working with a frail or elderly population or must receive appropriate training upon hire.
• Possesses and maintains current CPR certification.
• Proficient reading and writing comprehension skills .
• No more than 2 moving traffic violations during the previous 36 months . No driving under the influence (DUI) [Drug or Alcohol] conviction during the previous 5 years in a commercial or personal motor vehicle.
• Communicative, pleasant, and courteous. Must be able to interact effectively with PACE participants, caregivers, IDT, and PACE program staff.
• Able to keep accurate records; well organized.
• Entry level computer skills needed.
• Must be a licensed driver.
