Job summary

Bluegrass Care Navigators is seeking a PRN PACE Transportation Driver to support Kentucky participants in the Lexington and Frankfort regions. You will transport participants to and from the PACE Center, medical appointments, and home, while maintaining safety and professionalism.

This role requires a high school diploma or GED, a valid Kentucky driver's license with at least two years of driving experience, and CPR certification.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years old with a valid Kentucky driver's license and 2 years of driving experience.
• Minimum of 1 year of work experience transporting passengers.
• Experience with frail or elderly individuals or willingness to training on hire.
• Must possess and maintain current CPR certification.
• No positive drug or alcohol tests, pre-employment, random, post-accident, or suspicion-based.
• Able to keep accurate records; possess basic computer skills.

Responsibilities

• Transport PACE participants to and from their destinations, including home and the PACE Center.
• Interact with participants, caregivers, and PACE program staff in a courteous, professional manner.
• Maintain accurate records and ensure safety and organization during shifts.

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Service Coverage

This role is responsible for providing services in both the Lexington (LEX) and Frankfort (FFT) regions

Position Summary

The PACETransportationDriveris responsible forthe direct transportation of PACE participants to and from their destinations, including home, the PACE Center, medical providers, etc. Position requires constant interaction with PACE participants, caregivers, community agencies, medical providers and PACE program staff and leadership.

Position Qualifications

• High school diploma or general education degree (GED)required.
• Must be at least 18 years of age and have a current Kentucky driver’s license, witha minimum of twoyears’driving experience.
• At least one yearofworkexperience in transportingpassengers.
• Minimum of oneyear’sexperience transporting individuals with special mobility needs or must receiveappropriate trainingupon hire.
• Minimum of oneyear’sexperience working with a frail or elderly population or must receiveappropriate trainingupon hire.
• Possesses andmaintainscurrent CPR certification.
• Proficient reading and writing comprehension skills.
• No positive drug or alcohol testincludingpre-employment, random, post-accident, or reasonable suspicion.
• No more than 2 moving traffic violations during the previous36 months. No driving under the influence (DUI) [Drug or Alcohol] conviction during the previous 5 years in a commercial or personal motor vehicle.
• Communicative,pleasant,and courteous. Must be able to interact effectively with PACE participants, caregivers,IDT,and PACE program staff.
• Able to keepaccuraterecords;well organized.
• Entry level computer skills needed.
• Must be a licensed driver.
