Job Summary:

Registered Nurses (RNs) are responsible for providing high-quality, holistic care to patients in a variety of healthcare settings. Their main responsibility is to assess, plan, implement, and evaluate nursing care for individuals, families, and communities. To accomplish this goal, RNs perform a number of essential functions, including evaluating patients' physical and mental health status, analyzing test results, developing care plans, administering medications and treatments, and documenting observations and actions taken. Additionally, RNs often serve as liaisons between patients and other members of the healthcare team, communicating relevant information about each patient's condition, prognosis, and response to treatment. To ensure continuity of care, RNs coordinate patient transitions between departments, facilities, and levels of care, facilitating smooth handoffs and minimizing potential errors. They also educate patients and families about diseases, treatments, and prevention strategies, empowering them to take charge of their own health and participate fully in the healing process. Furthermore, RNs assume leadership roles within their organizations, mentoring junior staff, overseeing complex cases, and advocating for policy changes that benefit both patients and providers.

Licensed Practical Nurses (LPNs) are responsible for performing various nursing duties under the supervision of a registered nurse or physician. Their primary role is to provide basic medical care to patients in hospitals, clinics, long-term care facilities, and private homes. Some of the job duties of an LPN include recording and monitoring patients’ vital signs, administering medication and injections, maintaining accurate records of patients’ health information, collecting samples for laboratory testing, performing routine wound care, assisting with personal hygiene and grooming, and collaborating with other members of the healthcare team to develop individualized care plans. Additionally, LPNs may also be responsible for educating patients and families about proper self-care techniques and managing chronic conditions. Overall, the goal of an LPN is to promote optimal health outcomes by delivering quality, evidence-based nursing care while ensuring patient safety, dignity, and confidentiality at all times.

Key Responsibilities:

  • Serve as a resource for nursing & medical issues, overseeing infectious disease control, client hygiene, and related health issues under nursing leadership.
  • Perform nursing assessments per service line requirements, utilizing a high level of clinical competence.
  • Administer medications via various methods according to service line requirements, ensuring complete documentation.
  • Communicate with clients regarding medication questions & needs, staging refill requests to providers.
  • Room clients, verify data/medical history, and obtain necessary preliminary requirements.
  • Complete required documentation in a timely manner, including nursing assessments and client interactions.
  • Ensure medical & medication records are accurate and up-to-date, completing prior authorization paperwork as needed.
  • Assist with general lab & phlebotomy duties while following infection control precautions.
  • Interview clients and families to gather pertinent nursing & medical history.
  • Participate in all required meetings and perform other related duties as assigned.
  • Provide direct patient care, including assessing patient needs, developing care plans, implementing interventions, and evaluating responses.
  • Monitor and record vital signs to assess patients’ health statuses.
  • Maintain up-to-date, accurate documentation on patients’ charts and communicate effectively

    with other healthcare providers regarding changes in condition, treatment plan modifications, and progress reports.

  • Perform routine wound care, cleaning wounds, and reporting any signs of infection or complications.
  • Collect specimens for laboratory testing, following standard precautions to prevent contamination.
  • Collaborate with other healthcare providers to ensure comprehensive and coordinated patient care.
  • Educate patients and families about disease processes, self-care strategies, and community resources.
  • Ensure patient safety by adhering to protocols related to fall prevention and emergency response.
  • Uphold ethical principles, legal standards, and regulatory requirements governing nursing practice.

Education, Experience, and/or Credential Qualifications:

  • Valid licensure as a registered nurse (RN) or current licensure as a Licensed Practical Nurse (LPN) in the state of practice; one year of experience preferred for RN.
  • Completed graduation from an accredited practical nurse program for LPN.
  • Ability to read & interpret documents such as lab results and procedure manuals.
  • Strong communication skills to effectively interact with clients, families, and other healthcare providers.
  • Ability to follow direction and accept supervision.
