JOB SUMMARY

Reporting to the group homes' Residential Coordinator and under the clinical supervision of the Rehabilitation Director, the Residential Counselor provides direct services to promote clients' participation in community activities and develop skills for independent living. Responsibilities include assessing clients' rehabilitation needs, developing and monitoring individual rehabilitation plans, and providing crisis services, including intensive support and follow-up for discharged clients. The Residential Counselor also transports clients as needed.

The role involves ensuring clients develop and utilize skills that maintain a safe residential environment and documenting service delivery. Additionally, the Residential Counselor may provide basic health education in areas such as nutrition, exercise, dental care, substance abuse prevention, and communicable disease prevention.

RESPONSIBILITIES
  • Participate in assessing clients' rehabilitative needs and developing individualized plans
  • Coordinate and deliver services to support client goals for independent living
  • Assist with developing self-care, socialization, community awareness, mobility, and money management skills
  • Provide health education and disease prevention as privileged
  • Supervise residents in home-like settings, ensuring safety and compliance
  • Coordinate community integration activities
  • Communicate progress with staff and document services
  • Monitor medication management when privileged, ensuring safety and proper documentation
  • Maintain vehicle safety and documentation
  • Participate in discharge planning and follow-up services
  • Attend meetings and training as required
  • Ensure compliance with policies and maintain home safety standards
  • Perform additional duties as assigned
PHYSICAL REQUIREMENTS

The physical demands include operating a vehicle, traveling, standing, sitting, bending, reaching, lifting up to 30 pounds, and performing basic maintenance activities. Reasonable accommodations are available for individuals with disabilities.

REQUIREMENTS
  • High School Diploma or GED with at least two years of experience in a mental health residential or social service setting, or related post-secondary education
  • Bachelor's Degree or RN licensure with at least one year of rehabilitative service experience preferred
  • Excellent communication skills
  • Ability to work in a fast-paced healthcare environment
  • Experience with Microsoft Office Suite
