Job summary

Adoration Health is seeking a Home Health Registered Nurse for Gainesville, GA. Provide patient-centered, in-home care with a focus on independence and family involvement.

You will collaborate with an interdisciplinary team and contribute to comprehensive care plans in a supportive environment. The role requires an active RN license, at least 1 year of nursing experience, a valid driver’s license, and CPR certification.

Qualifications

• RN licensure in good standing in the state of practice and residence.
• Minimum of 1 year nursing practice in patient care; home health/geriatrics preferred.
• Valid driver’s license with driving record acceptable per policy.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychosocial needs of patients.
• Create home health care plans aligned to MD orders and patient goals.
• Direct nursing care: medications, treatments, and interventions.
• Provide pain and symptom management for patients.
• Educate and support families and caregivers; collaborate with interdisciplinary team.
• Maintain accurate documentation and participate in on-call rotation as required.

Responsibilities

As a Home Health Registered Nurse, You will:

• Assess/monitor physical, emotional, and psychological needs of patients
• Create home health care plans that align with MD orders and the patient's goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with an interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by the local branch

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required
• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
