BAYLOR RN WILL BE REQUIRED TO WORK TWO (2) 11.5 SHIFTS ON SUNDAYS AND MONDAYS

RN WILL WORK 23 HOURS AND BE PAID FOR THE EQUIVALENT OF WORKING 34.5 HRS AND IS ELIGIBLE FOR FULL TIME BENEFITS

Job Summary: Under the supervision of the Clinical Manager, the RN field clinician is accountable for assessing patients for eligibility for homecare services, admitting eligible patients following the admission process, and managing patients from admission through transition into the community. Responsibilities may include admissions, revisits, resumptions of care, recertifications, and discharges to ensure safe and effective patient care. This position coordinates interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. These responsibilities are carried out in accordance with organization policies and procedures, as well as Federal, State, and accreditation regulations.

Specifications, Education: Equivalent education required for RI licensure, a bachelor’s degree in a relevant discipline is strongly preferred.
