Job summary

Eden Nursing & Rehabilitation is seeking a Baylor Licensed Practical Nurse (LPN) in Brackenridge, PA to provide direct patient care and support to our residents. You will assess needs, implement care plans, and coordinate with the healthcare team to ensure optimal outcomes.

You will mentor Care Assistants, document records, and participate in quality improvement efforts to continually enhance patient satisfaction and safety.

Qualifications

Responsibilities

• Provide direct patient care to our residents, including assessing, planning, implementing, and evaluating care plans

• Perform routine vital signs and monitor patient conditions

We are seeking a dedicated and compassionate Baylor Licensed Practical Nurse (LPN) to join our team at Eden Nursing & Rehabilitation (formally known as Platinum Ridge). As a valued member of our team, you will play a vital role in providing high-quality patient care to our residents, while upholding our commitment to excellence and compassion.

Shift: Full Time, Saturday & Sunday 7am-11pm.

Rate of Pay: $28-$32/hour

About the Role:

As a Baylor Licensed Practical Nurse (LPN), you will be responsible for providing evidence-based nursing care to our residents, under the supervision of our healthcare team. You will assess patient needs, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure optimal patient outcomes. Your expertise will also be utilized in mentoring and educating our Care Assistants and other nursing staff.
