GENERAL FUNCTION

Under the direction and supervision of the Wellness Director, the Program Instructor III-Chronic Disease Prevention programs will be primarily responsible for planning and delivering instruction and support of Chronic Disease Prevention (CDP) programs (including but not limited to the YMCA’s Diabetes Prevention Program, LIVESTRONG at the YMCA, Blood Pressure Self-Monitoring Program, Enhance Fitness and Moving for Better Balance) at the YMCA, by helping participants achieve their physical health and well-being goals.

KNOW HOW

A high school diploma, or its equivalent, and 2-4 years experience leading/facilitating classes and working with special populations is required. A minimum of 18 years of age. Knowledge of basic health and wellness principles and exercises, as well as healthy living behaviors is required. Knowledge of form and technique of strength training exercises and function of cardio equipment preferred. Experience or ability to work with patients’ physician’s instructions required and have the ability to address the unique needs and concerns of cancer survivors resulting from their diagnoses and treatments. Must have exceptional communication and organizational skills in order to support participants and maintain required documentation for program. Knowledge and experience with health behavior and challenges associated with making lifestyle changes. Verbally provide clear concise directions and visually inspect classroom for safety. Ability to maintain confidentiality of program participants. Maintain current certifications in HIPAA, CPR AED, First Aid, Safety & Risk Management, Child Abuse Prevention and New Employee Orientation or the ability to obtain within 60 days of employment. Certified (or must obtain within 90 days of employment and prior to instructing CDP course) required pre-requisites and certification for CDP.
