Qualifications

• Current unrestricted medical license in state of jurisdiction as required for clinical duties.
• Unrestricted DEA license for state of jurisdiction.
• Must be eligible to participate in Medicare.
• Medical degree (MD) or Doctor of Osteopathy (DO) from accredited institution.

Responsibilities
