Job summary

Open Sky Community Services is seeking a dedicated professional to provide person-centered supports in our community-based programs. You will respond to crises using established procedures, implement health and medication protocols, and assist individuals with transportation needs.

• Utilize crisis intervention techniques and follow established program and agency procedures to respond effectively during emergency or crisis situations.
• Implement individuals’ health, medical, and medication protocols in accordance with agency policies and applicable regulations.
• Assist individuals with accessing transportation and coordinating travel needs, as required.
• Review, complete, and maintain all required documentation in a timely and accurate manner.
