Qualifications

• Active, unencumbered LPN/LVN license in the practicing state (compact licenses must be transferred within 90 days)
• Current CPR certification (training assistance available post-hire)
• TB skin test within the last 12 months
• Preferred but not required: six months of hands-on nursing experience
• Reliable transportation

Responsibilities

• Provide one-on-one nursing care in patients’ homes

• Administer medications, treatments, and therapies
• Perform patient assessments and coordinate care
• Educate and support family members regarding health management
