Job summary

GoodShepherd Healthcare Providers is seeking a Home Care Aide to provide one-on-one in-home care aligned with each patient’s care plan. You will work directly with patients, forming genuine relationships and delivering personalized support in their homes.

Strong communication skills are essential, and prior home health or hospice experience is a plus. You will assist with daily activities, maintain care records, and support patients’ physical and mental well‑being through interaction and light

Qualifications

• Must be a certified Home Health Aide with proper credentials.
• Driver’s license required.
• Experience in home health or hospice is a plus.

Responsibilities

• Assist patients with activities of daily living (bathing, dressing, mobility)
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report to supervisor
• Build relationships with patients by talking with them and reading aloud to keep them mentally engaged
• Administer medications and immunizations as directed
• Engage patients in exercises or other activities to promote health and well‑being

Job Summary

We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities

Qualifications

• Graduated from an accredited Home Health Aide program
• High School Diploma or GED
• One year prior professional experience
• Driver’s license required
