Youre Family Home Care LLC is seeking a Home Care Aide to join our team. You will work directly with patients in their homes, following a one-on-one care plan with a compassionate, patient-centered approach. Strong communication skills are essential, and prior Home Health or Hospice experience is a plus.

The role emphasizes detailed, organized care, building genuine relationships with clients, and contributing to a healthy living environment tailored to patient needs.

Qualifications

• Own transportation is required.
• High School Diploma or GED is required.
• At least one year of professional care experience is preferred.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor.
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
• Administer medications and immunizations as directed.
