Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in Evanston, IL, including private homes and senior living communities. You’ll develop plans of care, provide evidence-based therapy, and document in FOX’s EMR while maintaining autonomy over your schedule.

Clinicians may use telehealth to extend access and support more patients, enjoying a flexible, meaningful role in a collaborative, interdisciplinary team focused on aging in place.

Qualifications

• Current Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Graduate from an accredited Physical Therapy program
• Strong ability to work in home- and community-based settings

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Develop individualized plans of care and deliver evidence-based treatment
• Deliver therapy sessions that support mobility, independence, and functional outcomes
