Qualifications

• 1+ years of experience in post-acute/SNF/LTC settings.
• Prior post-acute, long-term care or skilled nursing facility experience.

Responsibilities

• Provide high-quality management for patients with chronic diseases in post-acute care.
• Collaborate with the attending physician and interdisciplinary team to coordinate care.
• Engage with Director of Nursing and social worker to improve patient outcomes.
