Job summary

MedCap Health seeks a part-time LongTerm and Post-Acute Care APP to deliver compassionate, autonomous care for patients with acute and chronic diseases. You will work Monday through Friday, day shifts, with no overnights, and collaborate with an attending physician and multidisciplinary team.

The ideal candidate is a board-certified geriatric, family, or adult NP with 3–5 years’ RN or post-acute pulmonary experience and 1+ year in post-acute settings.

Qualifications

• Board-certified NP (geriatric, family or adult) required.
• 3–5 years RN or post-acute experience in pulmonary care.
• 1+ year post-acute, long-term, or skilled nursing FEU experience in pulmonary.

Responsibilities

• Manage patients with acute and chronic diseases.
• Collaborate with attending physician for high-quality care.
• Coordinate with Director of Nursing and social workers for holistic care.
