Qualifications

• Minimum of 3 years of clinical experience, including primary care.
• Experience in integrated care or harm reduction models preferred.
• Comfort working with high-risk populations (PTSD, severe anxiety, SUD, disabilities).
• Strong collaboration, communication, and problem-solving skills.

Responsibilities

• Conduct in-depth health assessments and develop individualized care plans.
• Provide holistic medical care, addressing physical, mental, and social needs.
• Coordinate with Behavioral Specialists, Care Coordinators, and nursing staff.
• Assist patients with non-medical needs such as securing housing, employment, or transportation.
• Maintain patient engagement without volume-based pressure—no set daily patient quota.
