Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to provide one-on-one OT services in patients' homes and in senior living communities across Ohio. The Geriatric House Calls model emphasizes autonomy, flexibility, and meaningful outcomes, with optional telehealth to extend care.

You will evaluate, plan, and deliver evidence-based OT, document in our EMR, and collaborate with an interdisciplinary team to promote safety and aging in place.

Qualifications

• Valid Occupational Therapist license in the state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in home or community settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
