Job summary
FOX Rehabilitation is seeking a licensed Occupational Therapist to join our Highland Heights, OH practice delivering one-on-one geriatric care in patients' homes and in senior living communities. You will conduct evaluations, develop plans, and provide evidence-based therapy while coordinating with an interdisciplinary team and utilizing flexible scheduling including telehealth when appropriate.

Qualifications
- Valid Occupational Therapy license in the state(s) of practice.
- Degree from an accredited Occupational Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
Responsibilities
- Provide one-on-one Occupational therapy to older adults in homes or senior living settings.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support mobility, independence, and overall wellness.
- Document effectively within Fox's EMR system following regulatory standards.
- Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Provide telehealth/virtual care when appropriate to extend access.
