Job summary

Millennium Physician Group seeks Speech and Language Pathologists (PRN) to join our Home Health Department serving Lee, Charlotte, and Collier Counties in Florida. You will assess, treat, and educate patients in their homes, helping improve communication and swallowing safety with evidence-based therapy.

Qualifications

• Florida SLP licensure is required.
• Minimum of 1 year experience in hospital, rehab, or skilled nursing facility preferred.
• Strong clinical, communication and interpersonal abilities; patient-centered approach.

Responsibilities

• Assess patients' speech, language, cognitive-communication, and swallowing abilities.
• Develop individualized treatment plans with goals for speech, language, fluency, voice, and swallowing.
• Provide direct therapy using evidence-based practices across modalities.
• Educate patients and families on strategies to improve communication and swallowing.
• Collaborate with physicians, occupational and physical therapists to plan care.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries.
• Adhere to HIPAA, Medicare, and related regulatory standards.
