Millennium Physician Group is seeking Speech-Language Pathologists (PRN) to join our Home Health Department serving Lee, Charlotte, and Collier Counties. You will assess and treat patients in the home, develop personalized plans, and collaborate with a multidisciplinary team to improve communication and swallowing safety.

Qualifications

• Current and valid Speech-Language Pathologist license in Florida.
• Minimum of 1 year of experience in hospital, rehab, or skilled nursing facility.
• Strong clinical skills and interpersonal abilities; patient-centered care.
• Reliable transportation to visit patients in their homes.

Responsibilities

• Patient Assessment: Conduct comprehensive evaluations of patients' speech, language, cognitive-communication, and swallowing abilities to identify strengths, weaknesses, and treatment needs.
• Treatment Planning: Develop individualized treatment plans that address specific patient goals, including speech sound production, language development, fluency, voice disorders, and swallowing difficulties.
• Therapeutic Interventions: Provide direct therapy services, such as articulation therapy, language therapy, fluency therapy, voice therapy, and swallowing therapy, using evidence-based practices.
• Patient Education: Educate patients and their families on strategies to improve communication skills, swallowing safety, and overall quality of life.
• Collaboration: Collaborate with other healthcare professionals, such as physicians, occupational therapists, and physical therapists, to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.
