Job summary

As a full-time Home Health RN, you will conduct assessments, develop care plans, perform post-hospitalization follow-ups, and document care in Epic, while maintaining Mercy's standards and safety policies. A valid Missouri RN license and BLS are required.

Qualifications

• Graduation from an accredited nursing program.
• Current RN license in Missouri.
• BLS certification required.
• Valid driver’s license and proof of auto insurance.

Responsibilities

• Provide in-home nursing care and case management.
• Perform assessments, care planning, and post-hospitalization follow-ups.
• Document all care in Epic and coordinate with Mercy Home Health team.
