Qualifications

• MD or DO degree from an accredited medical school
• Completed residency in Family Medicine or Internal Medicine
• Board Certified or Board Eligible (FM or IM)
• Active, unrestricted Florida medical license (or ability to obtain licensure prior to start)

• Provide outpatient primary care with emphasis on preventive care and chronic disease management.
• Support patient relationships and continuity of care in a physician-led organization.
• Collaborate with a multidisciplinary team to deliver high-value care and improve patient outcomes.
