Job summary

MedCap Health is seeking a full-time LongTerm and Post-Acute Care APP to reshape the future of post-acute care. You will join a multidisciplinary team, managing patients with acute and chronic diseases, collaborating closely with the attending physician to deliver high-quality care and improve quality of life.

Ideal candidates are board-certified in geriatrics, family, or adult nursing and have 3–5 years of RN or post-acute GI experience, plus 1+ year in post-acute or skilled nursing facilities.

Qualifications

• Experience in post-acute care settings with gastroenterology focus.
• Ability to collaborate with physicians, nurses, social workers, and leadership.
• Proven ability to manage patients with acute and chronic conditions.

Responsibilities

• Manage patients with acute and chronic GI conditions in post-acute care.
• Collaborate with attending physician for high-quality care.
• Coordinate with Director of Nursing and social workers for comprehensive care.
