Job summary

MedCap Health in the United States is seeking a Long-Term and Post-Acute Care APP (NP/PA) for a flexible, full-time/part-time role. The position offers autonomy, a Monday–Friday day schedule with no nights or weekends, and a contracted salary of $150,000–$200,000 per year plus benefits.

You will manage patients with acute and chronic diseases, collaborate with attending physicians, and work closely with the Director of Nursing and social workers to deliver high-quality, compassionate care.

Qualifications

• Board-certified Nurse Practitioner with active state license
• 3–5 years RN or post-acute role with pulmonary focus
• 1+ years post-acute, long-term, or skilled nursing facility experience in pulmonary

Responsibilities

• Manage patients with acute and chronic pulmonary diseases
• Collaborate with attending physician to provide high-quality care
• Coordinate with Director of Nursing and social workers for cohesive care
