Job summary

Northern Light Home Care and Hospice Waterville is seeking a Physical Therapist to deliver home-based services with independence and accountability for patient outcomes. The role involves assessment, planning, and coordination of a home care treatment plan in collaboration with physicians, patients, and families.

You will supervise PTAs, students, and Home Health Aides, document care, and participate in team-based care while adhering to Maine PT practice acts and NLH standards.

Qualifications

• Current licensure to practice as a Physical Therapist in the State of Maine.
• BLS for Healthcare Providers certification is required to be obtained prior to independent visits.
• Valid driver's license with reliable transportation and agency required auto liability insurance.

Responsibilities

• Plan and implement individually designed programs of treatment to maintain, improve, or restore physical functioning.
• Perform initial exam, identify problems and set goals for reevaluation.
• Document care and billing within the EMR accurately and timely.
• Provide clinical supervision for home health aides, students, and PT/PTA.
• Coordinate with physicians, patients, and family to develop discharge plans.
