Job summary

Medley Health is seeking a compassionate Physician or Nurse Practitioner to provide on-site physiatry and pain management services across skilled nursing and rehabilitation centers. The role offers independent visits with a high degree of autonomy and collaboration with interdisciplinary teams to improve function and mobility for post-acute patients.

The position supports a flexible schedule with no nights or weekends, with compensation based on credentials and experience.

Qualifications

• MD or DO; Board Certified or Board Eligible in Physical Medicine & Rehabilitation (PM&R) preferred, or relevant experience in pain management, MSK, or rehab medicine
• Active New York medical license
• Experience in post-acute care, rehabilitation, pain management, or related settings preferred
• Strong clinical judgment, documentation, and collaboration skills

Responsibilities

• Perform comprehensive physiatry/rehabilitation evaluations and follow-up visits in SNF/Rehab settings
• Diagnose and manage musculoskeletal, neurologic, and functional limitations common in post-acute populations
• Develop and manage individualized treatment plans focused on functional restoration and mobility goals, pain management strategies, and coordination with PT/OT/ST plans of care
• Monitor patient progress and adjust care plans as clinically appropriate
• Document assessments and progress notes accurately and in a timely manner
• Communicate recommendations to facility clinical teams and assist with discharge planning when needed
