Job summary

Medley Health is seeking a compassionate Physician or Nurse Practitioner to provide on-site physiatry and pain management services across assisted living facilities in a part-time capacity.

You'll work independently with strong administrative support, collaborating with interdisciplinary teams to help residents regain function, improve mobility, and manage pain while enjoying autonomy and no night or weekend shifts.

Qualifications

• MD/DO, Board Certified or Board Eligible in PM&R preferred, or relevant experience in pain management, MSK, or rehab medicine.
• Active New York medical license for MD/DO.
• NP license (Adult-Gerontology preferred; FNP with relevant experience).
• Subacute Rehab experience is required.

Responsibilities

• Perform comprehensive physiatry and rehabilitation evaluations and follow-up visits in assisted living settings.
• Diagnose and manage musculoskeletal, neurologic, and functional limitations in older adult populations.
• Develop individualized treatment plans focused on functional restoration, mobility, and pain management.
• Coordinate with PT, OT, and ST teams on plans of care.
• Document assessments and progress notes accurately and timely.
• Communicate recommendations to facility clinical teams and assist with care transitions as needed.
