Job summary

OnSite Medical Solutions is seeking compassionate nurse practitioners to perform consultations in nursing homes, assisted living, and home settings. Qualifications

• MSN advanced practice certification (FNP/ANP/AGNP/AGACNP/AGPCNP/ACNP) required.
• Active Nursing License in South Carolina.
• DEA registration preferred or required depending on scope.

Responsibilities

• Round at assigned facilities and address acute, subacute, and primary care needs of older adults.
• Collaborate with OnSite team, facility staff, residents, and families to coordinate care.
• Proactively follow up with patients and assess response to interventions.
• Provide education on best practices for geriatric care.
• Incorporate non-pharmacological approaches whenever appropriate.
