Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist in Crystal Lake, IL to deliver one-on-one care in homes and senior living environments. You will assess, plan, and provide evidence-based therapy to older adults, focusing on independence and quality of life.

Join a clinician-led team with autonomy to schedule sessions, collaborate with an interdisciplinary group, and sometimes offer telehealth to extend access and continuity of care across communities.

Qualifications

• Valid SLP license in state of practice or eligibility to obtain.
• Degree from an accredited SLP program and CCC preferred.
• Current CPR certification and EMR proficiency.

Responsibilities

• Provide one-to-one Speech Language Pathology to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
