Job summary

Fox Rehabilitation in Council Bluffs, IA, is seeking licensed Speech Language Pathologists to provide one-on-one care in homes or Senior Living Communities. You will lead evaluations, create individualized plans, and deliver evidence-based treatment to improve mobility and quality of life.

Qualifications

• Requires valid SLP license in state of practice or eligibility.
• Degree from an accredited SLP program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• CCC-SLP Certification (ASHA).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or Senior Living Communities.
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility and independence.
• Document accurately in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
