Job summary

FOX Rehabilitation is growing in Valley Stream, NY and is seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in homes or senior living communities through our Geriatric House Calls™ model.

Role offers autonomy to create and manage schedules, opportunities to use telehealth, and meaningful patient outcomes while collaborating with a multidisciplinary team. In NY, base pay is competitive with potential for growth.

Qualifications

• Licensed Speech Language Pathologist in NY or eligible to obtain license.
• Master's degree in Speech-Language Pathology from an accredited program.
• Current ASHA CCC credential.
• Proficient with EMR systems and CPR certification.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document care accurately within the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists as part of an interdisciplinary team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
